Companions & Advisors:

Cassandra - Friendly. Planning on rebuilding the Seekers of Truth after learning more about the order's history.
Solas - Romanced. Allowed him to kill the mages keeping his spirit friend hostage, and allowed him to remove her vallaslin. Went missing following the final battle.
Varric - Friendly. Returning to Kirkwall following the fall of Corypheus to help with relief efforts.
Iron Bull - Friendly. Saved the Chargers and became Tal-Vashoth. Remaining with the Inquisition.
Dorian - Friendly. Plans to return to Tevinter in the future, but for the time being wants to remain with the Inquisition and his 'best and only friend' awhile longer.
Sera - Unfriendly. Clashed culturally with the Inquisitor. Announced intention to leave after the final battle.
Blackwall - Friendly. Planning to continue serving the Inquisition.
Vivienne - Unfriendly at first, friendly after delivering the heart of the snowy wyvern. They have their differences, but eventually came to understand one another's points of view.
Cole - Friendly. Chose to become more human, with Varric's help. Staying on with the Inquisition.

Cullen - Friendly. Broken of his lyrium addiction. Remaining to command the Inquisition's forces.
Josephine - Friendly. Assisting her family in becoming stable once more now that assassins have been dealt with.
Leliana - Friendly, softened. Supported to become Divine, which she achieved.

Story Arc Choices:

Wrath of Heaven - Denied being chosen by Andraste.
In Hushed Whispers - Aided the mages and recruited Dorian. Time travel shenanigans followed by offering the mages a full alliance. Got scolded.
Here Lies the Abyss - Stopped the ritual, left Stroud in the Fade. Exiled the Grey Wardens.
Wicked Eyes & Wicked Hearts - Collected plenty of blackmail, but not quite enough. Celene was saved, Florianne was killed. Celene rules alone. Got a private dance out of Solas.
What Pride Had Wrought - Respected tradition and allied with Abelas and the sentinels. Drank from the Well because Solas refused and allowing a shemlen to do so was out of the question.
Doom Upon All the World - Killed Corypheus. Leliana made Divine. Everyone went home and partied? Except Solas, who noped the hell out.
